QUALIFYING CRITERIA
Patient must be 5 years old or older
Patient or Parent/Guardian must provide
consent to care by completing the
Consent Form
*This program or event is not presented, supervised, or
sponsored by the Los Angeles Unified School District.
The District assumes no liability or responsibility for any
loss or injury arising out of participation and is merely
permitting this event to take place at this facility because
of the possible interest of students and/or community
